About Your Appointment

We strongly encourage all of our patients to arrive early for appointments.  If you are a new patient, please plan to arrive at least 20 minutes prior to your scheduled appointment.  The following list will help save time, and give us the important information we need to provide you with the best possible care.

What to Bring:

• Insurance Cards
  It’s important to bring your insurance card because health plan benefits and financial obligations vary depending on specific coverage.
• Copayment
  Please remember that any applicable co-payment is due at time of service. 
• Medication List
  Please bring a list of current medications, including dosage information, to every appointment. If you prefer, you may bring your actual medication bottles instead of a list.
• Identification
  Please bring two forms of identification, including one with a photo.
• Patient Forms
  You may print these forms and complete them before coming into the office. Click here
