About Your Visit

We encourage our patients to arrive early for appointments. If you are a new patient, please arrive at least 20 minutes prior to your scheduled appointment. The following list will help save time, and give us the important information we need to provide you with the best possible care.

Insurance Cards

Health plan benefits and financial obligations vary depending on specific coverage.

Copayment

Any applicable co-payment is due at time of service.

Medication List

Please bring a list of current medications, including dosage information, to every appointment.

Identification

Please bring two forms of identification, including one with a photo.

Medical and Immunization Records

A copy of your past medical and immunization records will help keep your chart up to date with your medical history.
